How to File a Complaint

Provide basic information –name, address, Email address, contact details of the complainant and respondent, product or service being complained

Narrate facts related to the complaint including relief prayer (Repair, Replace, Refund, Others) OR

Fill-out the Initial Complaint Form (available for download) or use the webform

Attach proof of transaction or all evidence necessary to support the claim

Submit complete set of documents

Wait for DTI action on the complaint / schedule of mediation

Attend the mediation set by DTI

If mediation is successful, sign an Agreement reached with the respondent

If failure of mediation, prepare for the Adjudication
